Please find the following forms to complete:
1. A “Dental Registration and History” form to be completed on both sides. If you have a list of medications, please feel free to hand your list to us and we’d be happy to take a photocopy for our files.
2. To comply with the state and federal privacy laws there is a copy of the office “Notice of Privacy Practices” to keep for your records and an “Acknowledgement of Receipt” for you to sign where indicated and return with your paperwork.
3. PRE-MEDICATION REMINDER: If you have any medical conditions that require you to pre-medicate with an antibiotic, (such as a Heart Murmur, Mitral Valve Prolapse, or Artificial Joints) please remember to take the medication as prescribed by your medical physician.
